Rape is an act of violence.
Any doctor would notice the “normal” little tearing in the vagina or anus after penetration, but a forensic doctor suspecting rape would look for clues of bonding around wrist and legs, signs on the skin , under the nails for scratches, signs of arousal, the presence of added lubrificants, and a full report to check for drugs and alchool.
Of course in not easy to have 100% certainty, but there are been many cases where they had enough evidence to convince a court of law that sex wasn’t consensual.
