How do cars breakdown, when there are dashboard lights?

I thought dashboard lights were always supposed to tell you when something is wrong. So why can the engine or transmission blow up? Does this only happen when people ignore the dashboard lights? I’m in the USA if it matters.

Let’s say there is a crack somewhere in the transmission, but doesn’t cause any problems until one day the crack is subjected to more stress than usual and fails, rendering the entire transmission out of service.

How exactly would you suggest the car be aware of this tiny vulnerability in the metal, and make you aware of it before it suddenly fails and destroys the transmission?

Of course there are a multitude of sudden mechanical failures that cannot be anticipated by the cars software to give you a “heads up”.
