You eat the pill.
Pill is in your intestine.
Worm is in holding on to your intestines. Worm eats stuff in your intestines. In this case the pill.
For the OTC products, the pill paralyses the worm.
It can’t hold on to your intestine.
You poop out worms.

Like many other things; it’s about finding something that harms A without harming B

In this case; it’s what paralyzes parasites but has no impact on host.  When parasites are paralyzed they can not stay attached to host intestine and can be passed normally
