How do medications have an “increased risk of death”? How do they increase the risk?

I’ve been failing at finding a concrete reason for some medications having an increased risk of death. Generally I see this for medications for dementia patients. But how does it increase a risk of death?

Medication is administered to improve the quality of life and in the process the quantity of life is diminished.
