I’ve been failing at finding a concrete reason for some medications having an increased risk of death. Generally I see this for medications for dementia patients. But how does it increase a risk of death?

The side effects listed for any given drug include all REPORTED side effects during a drug trial. That means if during a drug trial, someone dies, they consider death to be a side effect. They do not do anything to confirm that the drug caused the death.
You can imagine that when you are testing a drug for people on death’s door like those with dementia, the odds of someone dying during the trial are quite high.
