Let’s say the patient was shot in multiple places and is conscious. Once the medics get there, how would they know if the person is in shock (hypovolemic)? What are the signs they look for?

They just look for the basic symptoms of shock. The main symptom of shock is low blood pressure but it can also display with rapid, shallow breathing; cold, clammy skin; rapid, weak pulse; dizziness, fainting, or weakness.
Given that there really aren’t any treatments for _shock_ – you treat the condition that is causing the shock – the main goal is just to monitor vitals and ensure that no life-threatening symptoms develop.
A thready (weak) pulse and shallow breathing are usually good indicators when combined with that kind of injury.
A person losing a lot of blood is going to also become cold as their circulatory system begins to shut down and attempt to keep the organs alive by diverting blood away from the extremities.
