In movies and tv, when somebody’s smothered with a pillow, how do they do that safely without cutting away? Is there a way you hold the pillow? Do they just hold their breath?

Most of the time they hold their breath. If you actually time those scenes you will be hard pressed to find one lasting over 30 seconds.
They likely turn their head to side and film from an angle that doesn’t show that, but they could just hold their breath and have a special signal in case they need air.
