Is it just a matter of it falling off a conveyor belt somewhere? Left at a loading dock?

Sometimes there are multiple similar addresses and it goes to the wrong one, falls behind a machine, gets sorted into the wrong bag to the wrong destination, address gets damaged, gets stuck to another…There are millions of items shipped per day(Or were pre covid anyway) if something can happen it will have happened somewhere.
It can be that, it can be the address label falls off or is unreadable, it can be that an underpaid postman wants an afternoon off so rather than delivering the mail they dump it somewhere. There are basically a multitude of reasons.
