One of the initial humans who got HIV had many many many many many sexual partners, who had lots of casual sex with other people without using condoms, or who shared needles when using drugs with other people. As such, there were a ton of people who got infected at first, before anyone know that HIV was a thing. These people, primarily gay men, were getting sick and didn’t know how. Sexually transmitted diseases being fatal hadn’t really been a thing before then.

Basically, the hard part is that it’s a disease that can be spread by sexual activity, and getting people to not have sex is HARD. Convincing people in parts of the world where condoms never really were a thing that they should do something that reduces sexual pleasure to keep from dying is hard, as a lot of people don’t want to believe that they are personally at risk.
