This kind of reaponse can be caused by any number of conditions including low red blood cell count. It can also be caused by adrenaline which diverts bloodflow away from skin and towards useful things like muscles.

Lack of blood to the face… decreased blood flow. When you’re sick, your body redirects blood to the infected area to flood it with the healing cells. This causes decreased blood to the other areas (like your face), causing pale skin.

Anemia- When you are really sick for a long period of time, like cancer, kidney disease, etc your body stops producing blood cells.

Low blood pressure-blood flow to the face and extremities can be lower This can happen when cold or sick

Hypoxia- Low blood oxygen levels. When you’re sick your body is using up more oxygen to fight infection. and your lungs may not be absorbing oxygen as well

All 3 of these can cause someone to look pale in the face
