Aspirin and ibuprofen are both in a drug class called “NSAIDs,” which stands for “non steroid anti inflammatory drug.” It’s just a really long way of saying they help with inflammation.

Paracetamol/acetaminophen does not have any anti inflammatory effects. It works primarily on an a couple of enzymes in the brain. We don’t know specifically how it counteracts pain, we just know that it does. There’s a lot about medications that we don’t know all the specifics on how it works, we just know that it does.

If you sprained your ankle, ibuprofen/aspirin/other NSAIDs would help your pain by directly working on and reducing the swelling and inflammation in your ankle. Paracetamol would just make you feel less of the pain.
