When a phone is connected to a bluetooth speaker there is two-way communication between the two devices which allows them to share volume settings.

On simpler devices, such as an older boombox fed from an aux cord, the phone (or other supply) would amplify the voltage (increase the volume of the signal) by some amount and then the speaker would have its own amplifier. Thus, the two settings multiplied together to give the final outcome.
