In the UK Ford started using them back in the 1980s and they are pretty common now.
You can just see the lines if you look closely, but they are so fine they don’t interfere with driving these days, early ones could be a little hazy at night.

They can, but there’s already hot air available at the front of the car, so it’s cheaper to just use that. Running ductwork to the rear windshield is more expensive than running wires.

My understanding is that it can, but ford(?) own the patent and so it’s only available on their cars.
