When your body temperature goes up either from a hot day in the sun or exercise, your body has to release that heat or else it will overheat (fever) and things will go south if it gets too hot.
Our bodies have sweat glands which release water (and other things like salt) when our body starts to feel hot. The sweat will be secreted to the surface of the skin via ducts and sweat pores. Your skin gets wet. The sweat evaporates releasing body heat into the air. Your body cools down.
