Because you headbutt with your forehead, which is very hard, and aim for something that isn’t the other person’s forehead

The only reason is because of technique. The part of the head that you headbutt with on purpose (upper part of the forehead) is pretty solid, and you want to hit a softer part of the other guys face like their nose.

If you mess up and accidentally go forehead to forehead directly both people will be in a similar amount of pain. Though based on momentum the person being hit will possibly be knocked backward/over

You are hitting the parts of their face which are squishy or brittle (nose cartilage, eye socket, eve teeth) with your forehead, which is very hard.

Hard things hitting soft or brittle things with momentum equals broken soft or brittle things.

If your forehead hits their forehead, it is damaging for both parties.
