Why is scratching the skin only pleasurable when it is itchy?

I reject the premise. Scratching can be pleasurable in other circumstances as well, it depends on the context. A light scratch as a massage would be pleasant to many. On the other side a heavy scratch, harder than what one likes for an itch is often found to be pleasurable in NSFW circumstances.
The irregular nature of a scratch may have a lot to do with why it disrupts thought which is helpful for all three cases that I described. Disrupting pain for the itch/massage, and getting one out of their head in more intimate moments.
It also may be that removing the impediment that caused itching is the biological impetus for the whole set as well.
