how do nightmares piece themselves together?

how is it that when you have a nightmare, it’s always various specific things that come together that your brain recognises as unpleasant or distressing, why does this happen?

From what I heard, nightmares are supposed to train you for a situation. Example:

You have an exam coming. The brain doesn’t recognize the danger but it recognizes the anxiety it causes you, so you get a nightmare
