Lactose is broken down by an enzyme called lactase. Lactose intolerance is simply caused by lack of lactase. Lactose-free milk is made by adding lactase directly to the milk, which breaks down the lactose for you.

Lactose is a complex sugar by using an enzyme you can break it into two simple sugars which are easier to digest. You should notice that lactose free milk is sweeter. That’s the two sugars you are tasting. It also lasts longer in the fridge but I don’t know why.
