Look up Lethwei on youtube – it’s a Burmese martial art similar to Muay Thai, except it also allows headbutts.
A proper headbutt typically involves taking the hard “helmet” part of your head and thrusting it against the delicate “mask” part of your opponent’s face. The swinging forehead to forehead headbutts you see in movies are just a cinematic convention.
