Pregnancy is a very complicated process. Probably one of the most complicated in terms of human biology. Many things can go wrong. But, as a starting point, a lot of the issues begin with the general health of the woman to begin with. Modern nutrition and medicine helps by ensuring a better starting point to begin with. Then the advanced monitoring tools (ultrasound etc) plus diagnostics like measuring blood pressure, composition, chemistry helps identify early problems. Still not every problem is avoidable.
In most modern countries, maternal mortality rates have seen a reduction in the neighborhood of 99% since 1900. Maternal deaths due to pregnancies fell from about 1% of pregnancies in 1990 to around 0.01% today.
