Some of the most common causes of death during childbirth (so excluding infections that set in after) are bleeding, extremely elevated blood pressure, and blood clots.

Modern medicine has dramatically reduced instances of these by simply having doctors who understand the conditions and having medications and tools to treat them. This means doctors who can quickly recognize bleeding and fix it (and give blood transfusions if necessary) or to give medications to reduce blood pressure or break up clots. Modern hygiene also means doctors can safely perform c-sections to get the baby out quickly if there’s a risk for any of these issues, or if these issues start to occur.

You didn’t mention deaths *after* childbirth, but I’ll briefly address that because in the past, deaths after childbirth caused by infection were very common. modern hygiene, again, has made the risk of infection much lower, and we also have antibiotics to treat infections if they do occur.

It should be noted that deaths during and related to childbirth are sadly much more common in poor countries than in rich ones. Poor countries often don’t have access to modern facilities with proper hygiene, medications, and medical instruments to make birth safer. Even within rich countries, there’s often a notable difference in maternal deaths between rich and poor areas.
