What happens when cemeteries become full?

I live in a large city and often walk by cemeteries surrounded by buildings. What happens to those cemeteries when they are at capacity?

What is supposed to happen is that part of the money paid for being buried there goes to a trust fund, and when the cemetery is full, the trust fund should be at a point where just the interest is enough money to fund basic groundskeeping/maintenance.
