Why are you not supposed to take some meds on an empty stomach? Why does that play a role at all?

There was a medicine my dr gave me for my acne, she said now whatever you do, dont eat when you take this. I was like okay, gotcha! I would take it when I woke up and wait to lunch to eat but I would always have horrible stomachs pains soon after the pill. Well I stuck with it and a few weeks later I went to the follow up and i told her my stomach hurt and i did what she said and didn’t eat. she was like no i told you to make sure to eat with it. No i dont think she did but that’s why my stomach was hurting. Didn’t hurt at all afterwards when i started eating in the morning with my pill.
