Depending on the medicine in question and how strong the stomach is of the person taking it, it may not matter.

But as a general rule of thumb, having food in the stomach when you take a pill makes it so you absorb the medicine in that pill more slowly, since it is also being absorbed by the food in your stomach. And for large and very solid pills, it helps keep the stomach from being irritated buy the pill being ground up .
